They can develop due to a wide range of underlying causes including allergic reactions, insect bites and stings, bacterial, viral, or fungal infections, and chronic inflammatory skin conditions such as eczema or contact dermatitis. Identifying the underlying cause is essential to selecting the most effective treatment and preventing recurrence or spread.

Bumps, welts, or papules Raised spots or welts on the skin — may be flat, dome-shaped, or fluid-filled depending on the underlying cause
Itching or tenderness Pruritus (itching) is one of the most common and distressing symptoms — may be mild or intense; scratching can worsen the rash and introduce infection
Blisters or pustules Fluid-filled or pus-filled lesions seen in certain conditions such as contact dermatitis, shingles, or secondarily infected rashes
Insect bite marks or sting sites Visible puncture marks, central sting points, or characteristic bite patterns surrounded by redness and swelling

Diagnostic Evaluation

Skin Scraping or Biopsy If Fungal or Parasitic Infection Suspected
A small sample of skin cells or tissue is collected and examined under a microscope or sent to the laboratory. Skin scrapings are used to identify fungal organisms such as those causing ringworm or athlete's foot, as well as parasites such as scabies mites. Biopsy provides a deeper tissue sample for more complex or atypical presentations that require histological analysis.
Patch Testing For Allergy-Related Rashes
A series of small amounts of common allergens are applied to the skin under adhesive patches, which are left in place for 48 hours and then assessed for reactions. Patch testing is the gold standard for identifying the specific substance causing allergic contact dermatitis, enabling targeted avoidance and more precise treatment planning.

Prescribed when a secondary bacterial infection develops in the rash — often indicated by the presence of pus, crusting, increasing warmth, or spreading redness. May be topical or oral depending on severity.

Follow-Up Guidance

Return if symptoms worsen or the rash spreads

If the rash continues to spread to new areas of the body, increases in size or intensity, or fails to respond to treatment within the expected timeframe, return for re-evaluation. This may indicate an incorrect initial diagnosis, a more resistant organism, or the development of a different underlying condition.

Watch for signs of secondary bacterial infection

Any skin rash can become secondarily infected, particularly if scratching has broken the skin surface. Return promptly if any of the following develop:

Pus or yellow/green discharge Increased redness or warmth Red streaking from the rash Fever or chills Crusting that is spreading
